After activating the plugin, I go to the admin Manage > Formbuilder and I only get a message "FormBuilder Management (v 0.57)" with nothing else on the page. I've changed permits to 777 and also deactivated/activated with no change. I am using WPMU but installed it in the wpmu/wpcontent/plugins/formbuilder dir (all other plugins work here).
