So, I think I figured out what the problem is.  A feature we recently added (the ability to send GET variables to the form in order to pre-fill in selected fields) has resulted in a bug on the script when used on WP blogs with the default permalink structure in place.

I'll have to work on fixing the bug and releasing a new version of FormBuilder as soon as possible.

I "solved" the problem in the meantime. Let me summarize what I've done:
I usually just copy the plugin files from project folder to project folder without checking for new updates (I'm using to many plugins unfortunately) but this time I noticed the wordpress alert, that there were a new version available. At this moment (1 month ago) I was using 0.42 without any errors or problems. Just thought a new version couldn't be too bad. Well, right after I activated the new version 0.54 suddenly everybody could see the error messages of all fields without doing anything, just loading the website which is bad, because they have to appear only if someone forgot to fill out a required field.

That's when I was asking for help. I followed Sheldon Kotyks advice and managed to delete all database tables and could downgrade to 0.53 - again same problem. Wanted to downgrade to 0.52 but saw that it wasn't recommended to use it so I installed 0.50 which still didn't change anything until I came back to where I've started: 0.42
This version works fine, without any bugs or errors I could complain about.

Same facts you could need: 
I'm using wordpress version 2.3.1 with the german language pack. Other plugins I've activated/in use:
+ External Links
+ Frame Buster
+ Google Analytics
+ Google XML Sitemaps
+ intouch extended
+ WP-ContactForm (de)
+ wpSEO
